What Is a Chemical Pregnancy?

A chemical pregnancy is one of the most emotionally difficult experiences for couples trying to conceive. It is a very early pregnancy loss — occurring before the fifth week of gestation — where a fertilised egg implants briefly but fails to develop. The pregnancy is detected biochemically (via a positive home pregnancy test or blood hCG result) but never reaches the stage where it is visible on ultrasound. For many couples, particularly those going through IVF, it can feel like a cruel step backwards. In my experience, understanding why it happens and what can be done to prevent recurrence makes a real difference.

How Common Are Chemical Pregnancies?

Chemical pregnancies are thought to account for up to 50–75% of all miscarriages. They often go unnoticed by women who are not actively testing for pregnancy early, as the loss typically coincides with, or follows shortly after, an expected period. With the sensitivity of modern home pregnancy tests, more women are now aware of these very early losses than in previous generations.

Causes of Chemical Pregnancy

The most common cause is chromosomal abnormality in the embryo — a random error in cell division that makes normal development impossible. This is not a reflection of anything the mother has done wrong. Other contributing factors include:

  • Poor egg or sperm quality
  • Low progesterone and an inadequate luteal phase
  • Thin uterine lining reducing the receptivity of the endometrium
  • Uterine abnormalities such as fibroids or polyps
  • Thyroid dysfunction
  • Immune system factors that interfere with implantation

Recurrent chemical pregnancies — where this pattern happens more than once — indicate that investigation and treatment are warranted. I would always recommend working with your GP or a reproductive specialist alongside TCM treatment in these cases.

TCM Understanding of Chemical Pregnancy

In Chinese medicine, the ability to conceive and sustain a pregnancy depends on the strength of kidney jing (essence), the quality of kidney yin and yang, and the stability of the thoroughfare and conception vessels. A chemical pregnancy often reflects a pattern where conception is possible but the environment is not yet sufficiently nourishing to sustain it. The most common TCM patterns I see in women with recurrent chemical pregnancies are kidney yang deficiency (insufficient warmth to support implantation), blood deficiency (an inadequate uterine lining), and liver qi stagnation affecting the thoroughfare vessel.

How Acupuncture Can Help

Research has shown that acupuncture improves uterine blood flow, supports progesterone production, and regulates the immune response — all of which are relevant to implantation and early pregnancy support. I typically treat patients weekly across the full cycle, with particular attention to the peri-implantation window. A number of studies have also shown that acupuncture given around the time of embryo transfer in IVF can improve live birth rates.

Chinese Herbal Medicine

Chinese herbal medicine can be used to strengthen the underlying deficiencies that contribute to chemical pregnancy. Formulas that tonify kidney yang and stabilise the pregnancy — such as Shou Tai Wan — are among the most researched in Chinese reproductive medicine. I prescribe pharmaceutical-grade granules from Sun Ten in Taiwan, tailored to each patient's individual pattern. Herbal treatment is generally continued until the end of the first trimester in women who achieve a sustained pregnancy.

Optimising Egg and Sperm Quality

Given that chromosomal abnormality in the embryo is the most common cause, improving the underlying quality of eggs and sperm gives each conception attempt the best possible chance. Egg quality is closely linked to mitochondrial function, which is supported by CoQ10, adequate antioxidant intake, and kidney yin nourishment in TCM terms. Sperm DNA fragmentation — a key driver of early pregnancy loss — can be meaningfully reduced with the right combination of antioxidant supplements and Chinese herbal treatment over a period of three months (one full sperm cycle).
