Acupuncture for Low hCG Levels in Early Pregnancy

Low or slowly rising hCG (human chorionic gonadotrophin) in early pregnancy is one of the most anxiety-provoking situations a woman trying to conceive can face. hCG is produced by the trophoblast cells of the developing embryo after implantation and should roughly double every 48–72 hours in a healthy early pregnancy. Levels that fail to rise appropriately may suggest threatened miscarriage or ectopic pregnancy — both of which require urgent medical assessment. Once serious causes have been excluded and a viable intrauterine pregnancy confirmed, traditional Chinese medicine has a long history of supporting vulnerable early pregnancies.
Urgent safety note
If you have low or slowly rising hCG, the immediate priority is urgent assessment by your GP, early pregnancy unit, or A&E to exclude ectopic pregnancy. Signs that require immediate medical attention include severe one-sided abdominal or pelvic pain, shoulder-tip pain, heavy bleeding, dizziness, or fainting. Ectopic pregnancy is a potential medical emergency and cannot be managed by complementary therapies. Do not delay seeking medical assessment — call your early pregnancy unit or 111, or go to A&E if symptoms are severe.

What is hCG?
Human chorionic gonadotrophin (hCG) is a hormone produced by the placenta after an embryo has implanted into the uterine lining. It is the hormone that home pregnancy tests detect in urine. hCG has several critical roles in early pregnancy:
- Maintains the corpus luteum so that it continues producing progesterone — essential for holding the pregnancy until the placenta takes over progesterone production around week 10
- Supports implantation and placental development
- Modulates maternal immune tolerance of the embryo
- Stimulates the thyroid gland (which is why thyroid issues can emerge or worsen in early pregnancy)
Normal hCG levels and doubling time
In a healthy early pregnancy, hCG should approximately double every 48–72 hours until it peaks around 8–10 weeks. Single hCG readings are much less informative than the trend over time — a single "low" number may simply mean the pregnancy is earlier than calculated. What matters most is how the level is changing:
- Doubling every 48–72 hours — reassuring pattern for an ongoing pregnancy
- Rising but slowly (less than doubling) — concerning for ectopic or non-viable pregnancy; needs close monitoring
- Plateauing or falling — suggests pregnancy loss or resolving ectopic
- Very high or rapidly rising — may suggest multiple pregnancy or rarely molar pregnancy
Once hCG reaches approximately 1,500–2,000 mIU/mL, a transvaginal ultrasound should be able to visualise a gestational sac in the uterus. If levels are above this threshold but no intrauterine pregnancy is visible, ectopic pregnancy must be excluded.
Causes of low or slow-rising hCG
Several possibilities must be considered:
- Earlier gestation than calculated — inaccurate dates are the most common explanation for apparently "low" levels at a given stage
- Ectopic pregnancy — must be excluded urgently
- Threatened miscarriage — may still resolve with a viable ongoing pregnancy
- Missed or inevitable miscarriage — embryo has stopped developing or is being lost
- Chemical pregnancy — very early pregnancy loss, often before ultrasound visibility. See my article on chemical pregnancy
- Blighted ovum (anembryonic pregnancy) — gestational sac develops but no embryo
TCM understanding of early pregnancy
In TCM, the ability to sustain an early pregnancy depends on the strength of Kidney yang — the warming energy that holds and nourishes the implanted embryo — and the nourishing capacity of the Kidney jing and Blood. Low hCG and threatened miscarriage may reflect:
- Kidney yang deficiency — insufficient warming energy to hold and sustain the implanting embryo
- Kidney jing deficiency — diminished foundational reproductive essence; associated with advanced maternal age or diminished ovarian reserve
- Blood deficiency — insufficient nutritive capacity to nourish the embryo
- Qi deficiency — Spleen qi unable to hold the pregnancy, often seen with recurrent early loss
- Blood stasis — impaired blood flow to the uterus and placenta
Acupuncture support in early pregnancy
Acupuncture in the luteal phase and early first trimester supports progesterone production, improves uterine blood flow, and activates the Kidney yang energy that sustains the pregnancy. Research has shown that acupuncture during IVF cycles improves implantation and clinical pregnancy rates, and that acupuncture in early pregnancy may reduce miscarriage risk — particularly in women with a history of recurrent loss.
Acupuncture points are selected carefully in early pregnancy. Pregnancy-safe points that tonify Kidney yang and Spleen qi are used; points traditionally contraindicated in pregnancy (SP 6, LI 4, BL 60, GB 21, lower abdominal points) are avoided. Key points include KD 3 (Taixi), BL 23 (Shenshu), ST 36 (Zusanli), and BL 67 (used only in specific circumstances). Treatment is typically weekly through the first trimester for women with previous miscarriage or early pregnancy concerns.
Chinese herbal medicine
Chinese herbal medicine has a long tradition of stabilising early pregnancy. The classical foetus-stabilising formula is Shou Tai Wan (modified to the individual pattern) — combining Kidney yang tonics with foetus-calming herbs:
- Tu Si Zi (Cuscuta seed) — tonifies Kidney yang and calms the foetus
- Sang Ji Sheng (Mulberry mistletoe) — nourishes liver and kidney, calms the foetus
- Xu Duan (Dipsacus root) — tonifies liver and kidney, stops bleeding
- Du Zhong (Eucommia bark) — tonifies kidney yang
I prescribe pregnancy-safe formulas only, using pharmaceutical-grade granules from Sun Ten in Taiwan. Every herb is checked for pregnancy safety — strongly blood-moving herbs, toxic herbs, and those with uterine-stimulating action are excluded entirely. Herbs in pregnancy must only be prescribed by a practitioner registered with the Register of Chinese Herbal Medicine.
Lifestyle support in early pregnancy
- Rest — first-trimester tiredness is real and protective; honour it
- Avoid overheating — hot baths, saunas, and excessive exercise
- Avoid cold foods in TCM terms — cold drinks, ice, raw salads aggravate uterine cold
- Gentle warming foods — cooked vegetables, warm soups, warming grains
- Adequate protein — essential for trophoblast and placental development
- Continue prenatal vitamins — particularly folate or 5-MTHF, B12, iodine, vitamin D
- Reduce stress — stress suppresses reproductive hormone function
- Minimal physical exertion during the vulnerable early weeks, particularly around implantation and around the time of expected periods
Frequently asked questions
What counts as a low hCG level?
A single hCG number is less important than the trend. "Low" depends entirely on how pregnancy stage — normal hCG at 4 weeks can be 10–750 mIU/mL. What matters is whether levels are rising appropriately (doubling every 48–72 hours). Your early pregnancy unit will advise based on your specific numbers and symptoms.
Can acupuncture raise hCG levels?
Acupuncture does not directly raise hCG production. However, by supporting uterine blood flow, progesterone production, and Kidney yang, acupuncture may support the healthy trophoblast development that produces hCG. For a viable pregnancy, the combined effect can be meaningful; for a non-viable pregnancy, no treatment can change the outcome.
Can Chinese herbs prevent miscarriage?
Chinese herbal medicine has a long tradition of preventing miscarriage in women with vulnerable early pregnancies, and research supports this — particularly for recurrent miscarriage. Herbs cannot save a non-viable pregnancy (chromosomally abnormal embryos), but can support ongoing pregnancies where the cause of previous loss was functional.
When should I start acupuncture after a positive pregnancy test?
For women with a history of recurrent miscarriage or previous early pregnancy concerns, I typically continue weekly acupuncture from the two-week wait through the first trimester. For women with a straightforward pregnancy history, acupuncture is not routinely needed but is safe if desired.
Is it safe to have acupuncture in the first trimester?
Yes — first-trimester acupuncture is safe when performed by a practitioner experienced in pregnancy acupuncture. Specific points are avoided throughout pregnancy; the points used for early pregnancy support are all pregnancy-safe.
What if my hCG levels indicate miscarriage?
If the pregnancy is not viable, the focus shifts to supportive care through the miscarriage process and preparing for future pregnancy.
