Overview
San Wu Bei Ji Wan — the “Three Substance Emergency Pill” — is from Zhang Zhongjing’s Jin Gui Yao Lue. It is one of the most heroic and dangerous formulas in the materia medica, reserved for acute severe Cold accumulation in the gut with intestinal obstruction-like presentation. The three herbs — Ba Dou (croton seed, highly toxic warm purgative), Da Huang and Gan Jiang — combine to violently drive out cold accumulation downward.
In modern outpatient practice this formula is almost never used — acute severe abdominal pain with obstruction is managed surgically and medically. The formula is included here for completeness and historical interest. It belongs to the “reserve” classical formulas that experienced practitioners might use only in very specific circumstances.
I do not generally prescribe this formula in outpatient practice. It is presented here for educational reference.
TCM pattern
San Wu Bei Ji Wan is prescribed for acute severe Cold accumulation in the abdomen:
- Sudden severe lower abdominal pain
- Abdomen hard and resistant to pressure
- Severe constipation — no bowel movement, no gas
- Cold extremities
- Profuse cold sweating
- Strong, forceful body (not appropriate for weak or deficient patients)
- Tongue — pale, white slippery coat
- Pulse — deep, wiry, forceful

Cautions
Ba Dou is highly toxic. This formula is dangerous and is not used in modern outpatient practice. It is reserved for very experienced classical practitioners in specific historical settings. Modern Chinese hospital practice uses surgery and conventional medical care for acute intestinal obstruction.
Strictly contraindicated in pregnancy, in the elderly, in children, in weak constitutions, in any Heat pattern, in any Yin-deficient pattern.
