Overview

Tian Tai Wu Yao San — the “Top-Quality Lindera Powder” (named after the Tian Tai mountain region in Zhejiang where the best Lindera was grown) — is from the Song-dynasty Sheng Ji Zong Lu. It addresses Cold and Qi stagnation in the Liver channel of the lower abdomen, manifesting as the classical pattern of “cold hernia”: severe pain in the scrotum, lower abdomen or groin, often radiating, worse with cold and emotional stress.

I prescribe Tian Tai Wu Yao San as part of bespoke herbal formulas from pharmaceutical-grade granules sourced from Sun Ten in Taiwan.

TCM pattern

Tian Tai Wu Yao San is prescribed for Cold and Qi stagnation in the Liver channel:

  • Lower abdominal pain — often referred to the groin or scrotum
  • Cold sensation in the lower abdomen
  • Scrotal pain or testicular pain in men
  • Worse with cold exposure or stress
  • Better with warmth and pressure
  • Tongue — pale, white moist coat
  • Pulse — deep, wiry, slow

Key herbs

  1. Wu Yao — warmly moves Qi in the lower abdomen
  2. Xiao Hui Xiang (fennel seed) — warms and regulates Liver Qi
  3. Mu Xiang — moves Qi
  4. Liang Jiang (galangal) — warms the middle
  5. Qing Pi — breaks Liver Qi stagnation
  6. Bing Lang — descends Qi downward
  7. Chuan Lian Zi — moves Liver Qi and relieves pain
  8. Ba Dou (small dose) — warm purgative (some versions)

Formula actions

  1. Warms the Liver channel
  2. Moves stagnant Liver Qi
  3. Disperses Cold
  4. Relieves lower abdominal and scrotal pain

Conditions treated

  1. Inguinal hernia pain with cold pattern
  2. Hydrocele with cold pattern
  3. Testicular pain (orchialgia) with cold and Qi stagnation
  4. Chronic pelvic pain in men
  5. Period pain radiating to the lower abdomen with cold — see period pain
  6. Varicocele with cold pattern

Cautions

Scrotal or testicular pain needs medical assessment to exclude testicular torsion (emergency), epididymo-orchitis, hernia or cancer.

Some versions contain Ba Dou (croton seed, toxic) — ensure the modern preparation does not include this if used in non-emergency settings.

Not appropriate for Heat-pattern hernia (red, hot, painful with fever).

Not appropriate in pregnancy.
