Tong Xie Yao Fang — Painful Diarrhoea Essential Formula
On this page
Overview
Tong Xie Yao Fang — Painful Diarrhoea Essential Formula — is the classical Chinese herbal formula specifically designed for abdominal pain that precedes diarrhoea from Liver Qi overacting on the Spleen — the most common underlying pattern in stress-related irritable bowel syndrome. A focused, four-herb formula that simultaneously spreads Liver Qi, tonifies Spleen Qi, astringes the intestines and stops pain, addressing all four aspects of the pathological mechanism.
TCM pattern
Prescribed for Liver overacting on the Spleen (Gan Pi Bu He), characterised by: abdominal pain that precedes and is immediately relieved by diarrhoea, the pain and urgency recurring episodically (particularly after emotional stress), urgent loose stools, possible mucus in the stool, a wiry left and weak right pulse, and symptoms that clearly worsen with emotional tension or anxiety.
Key herbs
- Bai Zhu (Atractylodes macrocephala rhizome) — the principal herb; tonifies Spleen Qi, dries Dampness and strengthens the Spleen’s ability to resist Liver overaction
- Bai Shao (Paeonia lactiflora root) — nourishes Liver Blood, softens the Liver and alleviates cramping pain
- Chen Pi (aged Citrus reticulata peel) — regulates Qi, dries Dampness and relieves abdominal distension and pain
- Fang Feng (Saposhnikovia divaricata root) — expels Wind from the Spleen and Liver channels; raises Spleen Qi and dries Dampness
Conditions treated
- IBS with predominant diarrhoea (IBS-D) — the formula’s primary and most evidence-based indication
- IBS with mixed bowel habits (IBS-M) — often the most appropriate base formula for alternating diarrhoea and constipation
- Stress-triggered digestive complaints with cramping, urgency and loose stools
- IBS symptoms in Crohn’s disease and ulcerative colitis remission phase
Research evidence
Tong Xie Yao Fang is one of the better-studied classical Chinese herbal formulas in contemporary clinical research. A randomised controlled trial in Alimentary Pharmacology & Therapeutics (2018) found the formula significantly more effective than placebo in reducing IBS symptom severity scores, bowel urgency and abdominal pain in diarrhoea-predominant IBS. Multiple systematic reviews of RCTs have confirmed its effectiveness for IBS-D as both a standalone treatment and in combination with conventional medications.
