1. Overview
LV 4 — Zhōng Fēng (中封) — is an acupuncture point on the Liver meridian. Its English name, Middle Seal, reflects the point’s classical character and location. It is used regularly in acupuncture practice for a range of conditions.

2. Location
Anterior to the medial malleolus, in the depression medial to the tendon of tibialis anterior.
3. Actions
- Spreads Liver Qi and benefits the genital region
- Clears Damp-Heat from the Liver channel
- Activates the meridian and relieves ankle pain
4. Indications
- Genital pain, swelling and hernia from Liver Qi stagnation
- Urinary difficulty from Liver channel obstruction
- Orchitis and epididymitis in men
- Medial ankle pain and stiffness
5. Needling
Perpendicular insertion 0.5–0.8 cun.
Important: Acupuncture should only be performed by a fully qualified practitioner. All needles I use are sterile, single-use and disposed of in a clinical sharps container immediately after treatment.
6. Clinical notes
LV 4 is the Jing-River and Metal point of the Liver meridian. It is primarily used for disorders of the Liver channel in the genital region — orchitis, epididymitis, hernia and genital pain — where it is combined with LV 5 and LV 1. Its location at the medial ankle also makes it useful for medial ankle conditions.
