Overview

Jiao Tai Wan — the “Grand Communication Pill” — is attributed to Han Maozhou’s Ming-dynasty Han Shi Yi Tong. It is the simplest classical answer to the pattern of Heart and Kidney failing to communicate: Heart Fire flares above (insomnia, palpitations, mouth ulcers, agitation, vivid dreams) while Kidney Yang or Yin is deficient below (cold knees, lumbar ache, frequent urination). The formula uses Huang Lian to drain the Heart Fire downward and Rou Gui to lift the Kidney Yang upward, restoring the up-down axis between the two organs.

TCM pattern

Jiao Tai Wan is prescribed for Heart and Kidney failing to communicate (Xin Shen Bu Jiao):

  • Persistent insomnia, difficulty falling and staying asleep
  • Mind racing, vivid dreams
  • Palpitations, mild anxiety
  • Recurrent mouth ulcers, possibly with dry mouth
  • Lower-back ache, weak knees
  • Cold legs and feet despite warm upper body
  • Tongue — red tip with pale body, or split tongue
  • Pulse — rapid above, deep weak below

Cautions

The simple two-herb formula is usually used as part of a larger compound; rarely prescribed alone.

Not appropriate for pure Heat patterns, pure Yang-deficient cold without upper Heat, or chronic insomnia with marked depression that requires conventional assessment.

Persistent insomnia with daytime functional impairment, severe mood symptoms or suicidal thoughts requires mental health support.
