What is Ma Huang Tang?
Ma Huang Tang — Ephedra Decoction — is the foundational classical formula for an exterior excess wind-cold attack. It originates in the Shang Han Lun (Treatise on Cold Damage Disorders), Zhang Zhongjing’s clinical text from approximately 200 CE, and has remained one of the most studied prescriptions in Chinese medicine for almost two millennia. It is a small, sharp formula of four herbs whose actions are concentrated and direct — opening the surface, dispersing cold, easing wheezing and lifting body aches.
Because the chief herb Ma Huang (ephedra) contains sympathomimetic alkaloids, Ma Huang Tang is a strictly prescription-only formula in the UK. It is reserved for short-term use in acute presentations and is not appropriate for ongoing or self-administered use. The closely related but milder Gui Zhi Tang is used instead when the pattern is wind-cold deficiency rather than excess.
Ma Huang Tang TCM pattern
Prescribed for Tai Yang exterior excess wind-cold, characterised by: a sudden onset of marked chills with a low or moderate fever, an absence of sweating despite the fever, generalised body aches and stiffness (particularly across the upper back, neck and shoulders), headache, nasal congestion, possible wheezing with thin, watery, white sputum, a thin white tongue coat and a floating, tight pulse. The absence of sweating is the cardinal sign that distinguishes this pattern from Gui Zhi Tang’s deficient wind-cold pattern.
Key herbs
- Ma Huang (Ephedra sinica) — the chief herb; the strongest diaphoretic in the Chinese Materia Medica, opening the pores and dispersing wind-cold; also disperses Lung Qi and calms wheezing
- Gui Zhi (Cinnamomum cassia twig) — assists Ma Huang to release the exterior; warms the channels, eases body aches and harmonises the action of Ma Huang on the nutritive layer
- Xing Ren (Prunus armeniaca apricot kernel) — descends Lung Qi and assists Ma Huang to relieve wheezing and stop cough
- Zhi Gan Cao (honey-fried Glycyrrhiza root) — harmonises the formula, moderates Ma Huang’s strong dispersing action and protects the Middle Jiao

Conditions treated
- Acute wind-cold colds and influenza with marked chills, fever, no sweating and body aches — the formula’s primary classical application
- Cold-pattern asthma attacks with thin, watery, white sputum and wheezing, particularly when triggered by cold exposure
- Acute upper-respiratory infection in an otherwise robust constitution where the wind-cold has not yet penetrated to the interior
- Acute muscle and joint aches following exposure to cold and wind
Modifications of Ma Huang Tang produce several other key Shang Han Lun formulas: Da Qing Long Tang (Major Bluegreen Dragon) adds Shi Gao for internal heat with exterior wind-cold; Xiao Qing Long Tang (Minor Bluegreen Dragon) substitutes warming dispelling herbs for thin phlegm-fluid; Ma Xing Shi Gan Tang replaces Gui Zhi with Shi Gao for Lung Heat with wheezing.
Cautions
Contraindicated in: deficient wind-cold patterns (use Gui Zhi Tang instead), patterns with spontaneous sweating, Yin deficiency, hypertension, ischaemic heart disease, arrhythmia, hyperthyroidism, glaucoma, prostatic enlargement with urinary retention, pregnancy, and concurrent use of monoamine oxidase inhibitors (MAOIs), sympathomimetic drugs or stimulant decongestants. Not appropriate for chronic conditions. Use only for the duration of the acute exterior pattern; discontinue once sweating is achieved and the chills have resolved.
