Overview

Qi Bao Mei Ran Dan — literally “Seven Treasures, Beautiful Whiskers Pill” — is a famous Ming dynasty Chinese herbal formula recorded by Shao Yingjie in his medical text Yi Fang Ji Jie and popularised through wide use during the Qing dynasty. The formula was developed for premature greying of the hair, hair loss, weak lower back and knees, premature ageing and infertility arising from Kidney and Liver deficiency with Blood and Yin depletion. It is one of the principal classical formulas for tonifying Jing (Kidney essence) and nourishing Liver Blood — the two substances most directly responsible for healthy hair, vitality, libido and longevity in TCM.

The formula is built around He Shou Wu — the herb whose name literally means “Mr He's Black Hair”, after the legendary patient whose grey hair returned to black after taking it for an extended period — supported by six further tonic herbs that together nourish the Kidney, Liver and Blood and gently move stagnation so the tonic action is well absorbed.

I prescribe pharmaceutical-grade granules from Sun Ten in Taiwan, tested to the highest international quality and safety standards, and adjusted to each patient's individual pattern.

Composition

The classical formula consists of seven herbs — the “seven treasures” of the name:

  • He Shou Wu (prepared Polygonum multiflorum root, 30g) — the imperial herb of the formula; tonifies Liver and Kidney, nourishes Blood and Essence, blackens the hair
  • Fu Ling (Poria, 15g) — strengthens the Spleen, supports the assimilation of the rich tonic herbs and prevents the formula from becoming cloying
  • Niu Xi (Achyranthes root, 15g) — tonifies Liver and Kidney, strengthens the lower back and knees, guides the action of the formula downward
  • Dang Gui (Angelica sinensis root, 15g) — nourishes and invigorates the Blood, complements He Shou Wu in restoring hair colour and vitality
  • Gou Qi Zi (Lycium fruit, goji berries, 15g) — nourishes Liver and Kidney Yin, benefits the eyes, supports Essence
  • Tu Si Zi (Cuscuta seed, 15g) — tonifies Kidney Yang and Yin together, supports Essence and strengthens the lower back
  • Bu Gu Zhi (Psoralea fruit, 15g) — tonifies Kidney Yang, warms the lower burner, complements the Yin-nourishing core of the formula

The composition is a masterful balance: a Yin-tonifying and Blood-nourishing core (He Shou Wu, Dang Gui, Gou Qi Zi) is balanced by gentle Yang support (Tu Si Zi, Bu Gu Zhi), with Niu Xi guiding the action to the lower body, and Fu Ling protecting the digestive system from the richness of the formula. This combination tonifies without becoming cloying or generating Damp.

TCM Pattern Treated

Qi Bao Mei Ran Dan is designed for Liver and Kidney deficiency with Blood and Essence depletion. This pattern develops gradually as the deep reserves of the body decline — either through age, chronic illness, overwork, prolonged stress, repeated childbirth, or excessive sexual activity. The hallmark is the depletion of the substances that nourish the head and the lower back: hair greys prematurely or falls; the lower back and knees feel weak and ache; libido declines; tinnitus develops; and the patient feels gradually older than their years.

Clinical Indications

The characteristic presentation includes:

  • Premature greying of the hair — greying before the age of 40, or rapid greying after a period of severe stress or illness
  • Hair loss and thinning — particularly diffuse thinning rather than patchy alopecia; also useful in some forms of male and female pattern hair loss
  • Weakness and aching of the lower back and knees
  • Premature ageing — a clinical impression of fatigue, reduced vitality and an “older than the chronological age” appearance
  • Low libido in both men and women
  • Spermatorrhoea, premature ejaculation in men
  • Infertility and diminished ovarian reserve in women, particularly with low AMH and an early menopausal pattern
  • Tinnitus and dizziness with a hollow rather than throbbing quality
  • Soreness of the teeth and gums, loose teeth
  • Tongue: pale or normal in colour, often slightly dry, often slightly thin
  • Pulse: deep, thin, often weak especially at the rear (Kidney) positions

Conditions Commonly Treated

In modern clinical practice Qi Bao Mei Ran Dan is used for a range of conditions that present with the Liver-Kidney-Blood deficiency pattern:

  • Premature greying of the hair
  • Hair loss and thinning — including some forms of androgenetic alopecia, telogen effluvium and post-chemotherapy hair loss
  • Premature ovarian insufficiency and low AMH — supporting the broader formula plan for improving ovarian reserve
  • Male infertility with low testosterone or poor sperm parameters in the Kidney-Yin-deficient pattern — see male infertility
  • Chronic lower back and knee pain in the Liver-Kidney-deficient pattern
  • Recurrent miscarriage with deficient endometrial pattern
  • Erectile dysfunction with Kidney Yin and Yang depletion
  • Chronic fatigue with constitutional Kidney depletion

Research Evidence

The modern research on Qi Bao Mei Ran Dan is largely Chinese-language and focused on its principal indications: premature greying, alopecia, and male/female infertility. Studies have reported clinical improvement in hair density and pigmentation over 3–6 months of treatment, alongside laboratory findings of increased serum testosterone in some male studies, improved AMH and ovarian reserve markers in selected female studies, and antioxidant activity that reduces oxidative damage to hair follicles and germ cells. Mechanistic work on He Shou Wu specifically has identified anti-ageing effects via SIRT1 activation, telomerase modulation and modulation of the gut microbiota. As with all classical formulas, the trial quality is uneven and larger studies are needed, but the convergence of traditional indication with biomedically plausible mechanisms is encouraging.

Prescribing Notes

Where the patient's presentation has a stronger Yang-deficient quality (cold extremities, low libido with cold sensations, copious clear urination), I often add Yin Yang Huo (Epimedium) and Rou Cong Rong (Cistanche). Where the Yin-deficient and Heat features predominate (night sweats, hot palms and soles, thin red tongue), I combine with Zhi Bai Di Huang Wan to add Anemarrhena and Phellodendron. For patients with significant Blood deficiency on top of Kidney depletion — particularly women with very scanty or absent periods — Si Wu Tang can be layered alongside Qi Bao Mei Ran Dan in alternating cycles.

Cautions

Qi Bao Mei Ran Dan is contraindicated in acute Damp-Heat patterns, acute infections and the active phase of any inflammatory condition. Long-term high-dose He Shou Wu has been associated in case reports with reversible hepatotoxicity; the prepared form (Zhi He Shou Wu) used at standard doses within a balanced formula carries a much lower risk than raw He Shou Wu or high-dose single-herb preparations. Liver function should be checked at baseline and periodically during long-term treatment. As with all Chinese herbal formulas, Qi Bao Mei Ran Dan should only be taken under the supervision of a qualified Chinese herbalist (RCHM).
