1. Overview
Guā Lóu (栝楼) is the ripe fruit of Trichosanthes kirilowii, known in English as Chinese snake gourd or Trichosanthes fruit. The fruit shell (Guā Lóu Pí), the seed (Guā Lóu Rén) and the root (Tiān Huā Fěn) are also used separately for distinct actions. It belongs to the category of Cool herbs that transform Phlegm-Heat and is one of the principal herbs for chest oppression, sticky productive cough and Phlegm-Heat patterns in Chinese herbal medicine.

- Early-stage breast abscess and mastitis from Heat with Phlegm accumulation
4. Key formulas containing Guā Lóu
Guā Lóu is an important component of Gua Lou Xie Bai Bai Jiu Tang (Trichosanthes, Macrostem Onion and White Wine Decoction) for chest Bi syndrome with Phlegm obstructing the chest, and a number of formulas for Phlegm-Heat in the Lungs.
5. Modern research
Trichosanthes fruit and seed contain triterpenoid saponins, fatty oils, sterols and trichosanthin (a protein with antiviral and immunomodulatory activity studied separately). Pharmacological studies report anti-inflammatory, expectorant, mild laxative and antimicrobial effects, and protective effects on cardiac tissue in ischaemia models. Trichosanthin (derived from the root) has been investigated for antiviral and oncological applications, though it is not the form prescribed in standard herbal practice.
6. Cautions and contraindications
Contraindicated in cold-deficient diarrhoea and Spleen Yang deficiency. Traditionally considered incompatible with Wu Tou (aconite). The seed's oil content can loosen the stool; use carefully in patients with chronic loose stools.
