Huo Xiang Zheng Qi Wan (藿香正气丸) — Patchouli Rectify the Qi Pill

What is Huo Xiang Zheng Qi Wan?
Huo Xiang Zheng Qi Wan — Patchouli Rectify the Qi Pill — is a classical Song dynasty formula from the Tai Ping Hui Min He Ji Ju Fang (1107). It is one of the most widely used formulas in Chinese herbal medicine for summer colds, food poisoning, travellers’ diarrhoea and the kind of damp-cold gastroenteritis that occurs after eating cold food or being caught in a sudden downpour.
I prescribe Huo Xiang Zheng Qi Wan as part of bespoke herbal formulas from pharmaceutical-grade granules sourced from Sun Ten in Taiwan.
Huo Xiang Zheng Qi Wan TCM pattern
Prescribed for external wind-cold with internal Damp obstructing the middle Burner: chills, fever, headache, heaviness of head and body, chest oppression, nausea[12], vomiting, watery diarrhoea, abdominal pain, slimy white tongue coat and a soggy or floating pulse. Classic summer cold or food-poisoning pattern.
Key herbs
- Huo Xiang (Hb. Agastache, 3-15g) — chief herb; releases the exterior, aromatically transforms Damp and harmonises the middle
- Zi Su Ye (Fol. Perillae, 1-12g) — releases the exterior
- Bai Zhi (Rx. Angelica Dahuricae, 1.5-9g) — dispels Wind-cold and resolves Damp
- Ban Xia (Rz. Pinelliae Preparatum, 3-12g) — transforms Phlegm and stops vomiting
- Chen Pi (Per. Citri Reticulatae, 3-12g), Hou Po (Ginger-fried Cx. Magnoliae, 2-12g), Da Fu Pi (Per. Arecae, 1-12g) — move Qi and dry Damp
- Bai Zhu (Rz. Atractylodes Macrocephalae, 3-12g), Fu Ling (Poria, 3-12g) — tonify Spleen and drain Damp
- Jie Geng (Rx. Platycodi, 1.5-10g) — opens the upper Burner
- Sheng Jiang (Rz. Zingiberis Recens, 1-3g), Da Zao (Fr. Jujube, 1-5g), Gan Cao (Rx. Glycyrrhizae Preparata, 1.5-12g) — harmonise

How does Huo Xiang Zheng Qi Wan work?
Huo Xiang Zheng Qi Wan combines aromatic Damp-resolving herbs (Huo Xiang as principal, plus Zi Su Ye, Bai Zhi) with Spleen-supporting herbs (Bai Zhu, Fu Ling, Chen Pi, Hou Po) and harmonising herbs (Sheng Jiang, Da Zao, Gan Cao). The combination addresses Summer-Damp invasion of the digestive system — the classical TCM picture of acute Damp obstruction producing nausea, vomiting, diarrhoea, abdominal distension, headache and body aches typical of viral gastroenteritis, food-poisoning recovery, and travellers' Damp-pattern presentations. In TCM clinical practice the formula may help support symptomatic relief and recovery in self-limiting digestive infections alongside standard hydration and supportive care. The aromatic action also addresses the heavy-headed and brain-fog sensation of Damp accumulation.
Dosage and forms
- Pharmaceutical-grade granules — 4–6 g/day in 2–3 divided doses during acute presentations; treatment course typically 5–10 days. The most concentrated form.
- Patent pills — the convenient travel form; typically 8 pills three times daily. Easy to carry for travellers' insurance against gastrointestinal upset.
- Liquid preparation — the traditional liquid form, popular in southern China and now widely available; one vial 2–3 times daily.
I prescribe pharmaceutical-grade granules from Sun Ten in Taiwan, individualised within a bespoke prescription.
Cautions and contraindications
- Heat-pattern gastroenteritis — the formula is aromatic-warming; not appropriate for bloody dysentery, high-fever-with-foul-stool presentations, or marked Heat patterns. Different formulas (Bai Tou Weng Tang, Ge Gen Huang Qin Huang Lian Tang) target Heat patterns.
- Severe dehydration — medical assessment + oral or IV rehydration is the priority; the formula supports recovery but does not address dehydration itself
- Pregnancy — use cautiously, only under qualified herbalist supervision
- Persistent symptoms beyond 5 days — reassess; persistent gastroenteritis warrants medical evaluation

Comparison with related formulas
- Bao He Wan — targets food stagnation pattern (heavy meal-induced bloating, sour belching). Use for over-eating sequelae rather than for viral gastroenteritis.
- Ping Wei San — gentler chronic Damp-pattern digestive formula. Use for chronic baseline Spleen-Damp rather than acute summer-Damp invasion.
- San Ren Tang — for Damp-Heat invasion at the three Jiao levels (bronchitis with body aches and heavy head); use when the picture is more febrile than purely gastrointestinal.
Frequently asked questions
Is Huo Xiang Zheng Qi Wan the same as Huo Xiang Zheng Qi San?
Same formula in different delivery forms. “San” is the granule or powder form; “Wan” is the pill form. Composition is identical. Both are widely available; choose based on convenience.
Can I take it on travel for food-poisoning prevention?
Yes — it is widely used by Chinese travellers to hot-humid climates (South-East Asia, Africa, Latin America) as both a preventative travel kit item and an acute treatment if Damp-pattern gastroenteritis develops. The patent pill form is convenient to carry. Standard travellers' precautions still apply (safe water, well-cooked food, hand hygiene); the formula supports recovery if symptoms develop.
How quickly does it work in viral gastroenteritis?
Most patients notice symptomatic improvement (reduced nausea, less abdominal distension, more settled stool) within 24–48 hours of starting the formula. Full resolution of viral gastroenteritis typically occurs over 5–7 days regardless of treatment; the formula may help support recovery and reduce symptom burden during the course.
Can children take it?
Yes — at age-adjusted doses (typically 30–50% of adult dose). Particularly useful for school-age children with stomach upset, motion sickness or summer Damp-pattern viral illness. Always seek qualified herbalist supervision for paediatric dosing.
Does the formula help motion sickness?
Yes — the Damp-resolving and Stomach-harmonising action also helps motion sickness, particularly when nausea and heavy-headed sensation are the prominent features. Take 30–60 minutes before travel and continue every 4–6 hours during longer journeys. Can be combined with ginger and acupressure wristbands at PC 6 for synergistic effect.
