Overview

Pu Ji Xiao Du Yin — the “Universal Benefit Decoction to Eliminate Toxin” — is a Jin-Yuan dynasty formula from Li Dongyuan (1247) developed in response to an epidemic of mumps (then called “big-head wind”) that swept through northern China. The formula reportedly saved many lives during the epidemic, hence the name “Universal Benefit” — it was reportedly carved on stone tablets so it could be widely shared.

The pattern Li Dongyuan addressed is Wind-Heat toxin attacking the head and face, producing the swollen, red, painful neck and parotid glands of mumps, erysipelas of the face, or severe acute facial cellulitis. The formula combines Wind-dispersing herbs (Bo He, Niu Bang Zi, Jiang Can) with strong Heat-clearing herbs (Huang Qin, Huang Lian, Lian Qiao, Ban Lan Gen) and herbs that move Qi (Chai Hu, Sheng Ma) to lift the action up to the head.

TCM pattern

Pu Ji Xiao Du Yin is prescribed for Wind-Heat toxin in the head and face:

  • Swollen, red, painful neck or face — particularly parotid swelling (mumps)
  • High fever with chills
  • Severe sore throat
  • Headache
  • Dry mouth, thirst
  • Constipation
  • Dark scanty urine
  • Tongue — red, yellow coat
  • Pulse — rapid, forceful

Cautions

Suspected mumps, especially in adolescents and adults, needs medical assessment due to risk of complications (orchitis, pancreatitis, meningitis). MMR vaccination is the public-health prevention measure.

Acute severe facial cellulitis, particularly approaching the eye, needs urgent medical assessment and IV antibiotics — orbital cellulitis can cause blindness or cavernous sinus thrombosis.

Contains Jiang Can (silkworm), an animal product. Vegetarian patients can request modification.

Use cautiously in pregnancy — reduce or modify; the cold draining herbs can damage Spleen Yang.

Not appropriate for cold or Yin-deficient patterns.
