1. Overview

Huǒ Má Rén (火麻仁) is the ripe, dehulled seed of Cannabis sativa, known in English as hemp seed. It belongs to the category of Moistening laxative herbs that drain downward in the Chinese Materia Medica and is one of the safest and most widely used gentle laxative herbs in Chinese herbal medicine — particularly suited to dry constipation in the elderly, after illness, and in patients with deficient Yin or Blood.

Hemp seed has essentially no psychoactive activity: the THC-bearing parts of the plant are the female flowers and leaves, not the seed, and culinary and herbal-grade hemp seed is fully legal in the UK and Europe. The seed used herbally is the same as the hemp seed widely sold as food.

I prescribe Huǒ Má Rén as part of bespoke herbal formulas from pharmaceutical-grade granules sourced from Sun Ten in Taiwan — independently tested to the highest international quality and safety standards. Herbs are always combined with other herbs selected to match the patient’s individual TCM pattern. Online consultations are available for patients who cannot attend my clinic in person.

4. Key formulas containing Huǒ Má Rén

Huǒ Má Rén is the chief herb in Ma Zi Ren Wan (Hemp Seed Pill), the classical formula for dry constipation with mild heat in the intestines. It is also used in moistening combinations alongside Tao Ren (peach kernel), Yu Li Ren and Dang Gui.

5. Modern research

Hemp seed is rich in oils (around 30%), high-quality protein, fibre and minerals. The oil profile is favourable, with linoleic acid (omega-6) and alpha-linolenic acid (omega-3) in a ratio close to the dietary ideal, plus gamma-linolenic acid. Research supports the seed's lipid-modulating and laxative effects, and clinical trials of Ma Zi Ren Wan in functional constipation report meaningful benefit with a favourable side-effect profile compared with stimulant laxatives. THC content of properly processed hemp seed is negligible.

6. Cautions and contraindications

Use with caution in patients with chronic loose stools or Spleen-deficient diarrhoea. Excessive doses can cause cramping and loose stools. In line with the British Pharmacopoeia and food-supplement standards, only dehulled hemp seed from a reputable supplier should be used. There is no psychoactive effect at therapeutic doses; the THC content of properly processed seed is below detection thresholds.

Important: Chinese herbs should always be prescribed by a fully qualified herbalist who is a member of the Register of Chinese Herbal Medicine (RCHM).
